We have developed a new book to help patients, as they start to transition from children’s to adult services.

This specialist new resource is designed to bridge that space between adolescence and adulthood, helping young people build confidence and develop the self-management skills they need in their HPN care. It’s focused on growing independence, while recognising that what this looks like will be different for each individual.

The new book contains interactive elements such as journaling, goal-setting and reflection spaces.
